ASC provides the following forms to assist
parties involved in or considering an application under the Act.
These forms available for use by both Claimants and
Respondents:
Payment Claim – As most accounting
software produce a different style of payment claim
there is no standard form. However the information provided
should be reasonably detailed. At least
sufficient to ensure the Respondent will understand what
work is being claimed and when payment is due
The "front page" of the payment claim should
give totals with reference to attachments. The attachments
are where a more detailed description of the works and
amounts may be located.
Payment Schedule – A payment schedule
would normally follow the format and layout of the payment
claim to which it relates however there is no standard
format. It should state clearly and with sufficient
detail for the Claimant to be able ti understand why
payment is not in the full amount claimed
